Permit to work
- Permit types including hot work, cold work, confined space, work at height, excavation, electrical isolation (LOTO), lifting, line breaking and radiography
- Each type carries its required checks — gas test, isolation, fall protection, certification — with sensible defaults
- Issue is blocked until they pass: a satisfactory gas test within 30 minutes, isolation applied, fall protection with a rescue plan, a current certification
- Gas tests record O₂, LEL, H₂S and CO; lock and tag numbers are recorded, and a permit can't close while an isolation is still applied
- The raiser can't approve their own permit
Incidents, investigation and CAPA
- Incidents numbered per project, with lost days, body part affected and photos
- Structured 5-Why investigations — immediate, underlying and root causes, failed controls, lessons learned
- An investigation can't be completed without a root cause
